            Standard MRI scans are rarely able to show brain damage even when damage is present. High powered 3-Telsa MRI scanners often have the ability to show damage in a person who has suffered a concussion or mild brain injury. MRI protocols such as Diffusion Tension Imaging (DTI), Flair, Susceptibility Weighted Imaging (SWI) and Gradient Echo Series may be used to reveal the damage. Many 3-Telsa scanners are used primarily for research. Doctors can refer patients to VitalView Imaging in Omaha for clinical 3-Tesla brain MRI scans.

           Scientists from the University of Birmingham have shown that a brain-penetrating candidate drug currently in development as a cancer therapy can foster regeneration of damaged nerves after spinal trauma.

    &lt;span&gt;&#xD;

            The research, published today in

           , used cell and animal models to demonstrate that when taken orally the candidate drug, known as AZD1390, can block the response to DNA damage in nerve cells and promote regeneration of damaged nerves, so restoring sensory and motor function after spinal injury.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           The announcement comes weeks after the same research team showed a different investigational drug (AZD1236) can reduce damage after spinal cord injury, by blocking the inflammatory response. Both studies were supported by AstraZeneca’s Open Innovations Programme, which shares compounds, tools, technologies and expertise with the scientific community to advance drug discovery and development.

Now researchers at the Del Monte Institute for Neuroscience have found that kids who experience a traumatic brain injury (TBI), even a mild one, have more emotional and behavioral problems than kids who do not.

  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;

  &lt;p&gt;&#xD;

           “These hits to the head are hard to study because much of it depends on recall of an injury since the impacts do not all require a visit to a doctor,” said Daniel Lopez, a Ph.D. candidate in the Epidemiology program and first author of the study out today in NeuroImage. “But being able to analyze longitudinal data from a large cohort and ask important questions like this gives us valuable information into how a TBI, even a mild one, impacts a developing brain.”

The effectiveness of this approach in improving outcomes was demonstrated following brain injury, stroke or multiple sclerosis in mice. The system increases the number of regulatory T cells, mediators of the immune system’s anti-inflammatory response, in the brain. By boosting the number of T regulatory cells in the brain, the researchers were able to prevent the death of brain tissue in mice following injury and the mice performed better in cognitive tests. The treatment has a high potential for use in patients with traumatic brain injury, with few alternatives currently available to prevent harmful neuroinflammation.

      <title>Brain injury is a disease, not an event</title>

           In his recent white paper (Conceptualizing Brain Injury as a Chronic Disease), Brent Masel, M.D., medical director of BIAA, makes the case that brain injury is a disease, not an event. In support of that proposition, Dr. Masel presented stunning statistics showing, among many other things, that persons with traumatic brain injury have higher morbidity and mortality rates than their non-injured cohorts.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           A recently published paper entitled Neurodegenerative causes of death among retired National Football League players supports that proposition. The paper, authored by Lehman, EJ, Hein MJ and Barron SL, published online in Neurology (September 5, 2012), analyzed neurodegenerative causes of death especially Alzheimer’s, Parkinson’s disease, and ALS among a cohort of professional football players.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           The research demonstrated that neurodegenerative mortality of this cohort is three times higher than that of the general US population, that for Alzheimer’s disease and ALS it was four times higher. “These results were consistent with recent studies that suggest an increase risk of neurodegenerative disease among football players.” Neurology 2012; 79:1-1

During the past decade, the number of such studies has risen exponentially and continues to increase with no sign of abatement. A unifying theme can be deduced from this large body of research:

  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;

           DTI is an extremely useful and robust tool for the detection of TBI-related brain abnormalities. Overwhelming consensus of these studies is that low white matter FA is characteristic of TBI.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           We also found an overwhelming consensus that imaging abnormalities detected with DTI are associated with important clinical outcomes. This further validates DTI as a meaningful measure of clinically important brain injury.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           In summary, DTI provides a robust measure of clinically important TAI at cross-section, despite the variability inherent in characteristics of patients with TBI and injury mechanisms as well as study differences in data acquisition and analysis methods.”

      <title>Period Of Increased Vulnerability For Repeat Traumatic Brain Injury</title>

           Repeat traumatic brain injury affects a subgroup of the 3.5 million people who suffer head trauma each year. Even a mild repeat TBI that occurs when the brain is still recovering from an initial injury can result in poorer outcomes, especially in children and young adults. A metabolic marker that could serve as the basis for new mild TBI vulnerability guidelines is described in an article in Journal of Neurotrauma, a peer-reviewed journal from Mary Ann Liebert, Inc., publishers.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           In an Editorial, “The Window of Risk in Repeated Head Injury,” accompanying this article, John T. Povlishock, PhD, Editor-in-Chief of Journal of Neurotrauma and Professor, VCU Neuroscience Center, Medical College of Virginia, Richmond, states that recent studies of TBI in animal models have shown that while repeat injury can exacerbate structural, functional, metabolic, and behavioral responses, “these responses only occur when the injury is repeated within a specific time frame post-injury.”
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           “Specifically, this window of risk is greatest when the interval between injuries is short, hours to days, while any risk for increased damage is obviated when the intervals between injuries are elongated over days to weeks,” says Dr. Povlishock. It is not yet clear if these time periods of increased risk are age- or gender-specific or depend on the intensity of the initial injury.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           A consistent finding following TBI in both humans and animal models is a decrease in glucose uptake by the brain. Mayumi Prins, Daya Alexander, Christopher Giza, and David Hovda, The UCLA Brain Injury Research Center, Los Angeles, CA, simulated single and repeat (after 1 or 5 days) mild TBI in rats and measured cerebral glucose metabolism. They tested the hypothesis that the rats’ brains would be more vulnerable to the damaging effects of repeat TBI at 1 day post-injury, when glucose metabolism was still decreased, than at 5 days, when it had returned to normal levels.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           In the article, “Repeat Mild Traumatic Brain Injury: Mechanisms of Cerebral Vulnerability,” the authors propose that the duration of metabolic slowdown in the brain could serve as a valuable biomarker for how long a child might be at increased risk of repeat TBI.

      <title>PET Scans Now Identify Brain Disease In Living NFL Players</title>

           A group of researchers from UCLA are using PET scans to recognize chronic traumatic encephalopathy (CTE) in living NFL players. The scans show the presence of tau proteins linked to CTE. Before this, identification of the presence of this protein, which is also linked to Alzheimer’s disease, could only be confirmed via autopsy.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           Scientists from UCLA used a brain-imaging tool that was originally developed for examining neurological changes linked to Alzheimer’s disease. They put to work a chemical marker they made named FDDNP, which binds to deposits of amyloid beta “plaques” and neurofibrillary tau “tangles” (telltale signs of Alzheimer’s) – then they viewed it using a PET (positron emission tomography) scan. The researchers were able to identify where in the brain these irregular proteins built up. Participants received intravenous injections of FDDNP, while the researchers then performed PET brain scans and compared them to those of healthy men with comparable BMI, education, age, and family history of dementia. The scientists discovered that in comparison to healthy men, the NFL players had increased levels of FDDNP in the amygdala and subcortical regions of the brain – the areas that control emotions, behavior, memory, and learning. Participants who had a greater number of concussions had higher levels of FDDNP.
          &#xD;

           Study author Dr. Jorge R. Barrio, a professor of molecular and medical pharmacology at the David Geffen School of Medicine at UCLA, said, “The FDDNP binding patterns in the players’ scans were consistent with the tau deposit patterns that have been observed at autopsy in CTE cases.”

      <title>Children with brain injury often misdiagnosed</title>

           A new study by Tel Aviv University, Kaplan Medical Center and Shamir Medical Center (Assaf Harofeh) found that one in four children (25.3%) who have been discharged from the emergency room after a mild head injury are misdiagnosed and continue to suffer from persistent post-concussion syndrome for many years. This syndrome includes chronic symptoms such as forgetfulness, memory problems, sensitivity to light and noise, ADHD and even psychological problems and, instead of receiving treatment for the syndrome, they are mistakenly diagnosed as suffering from ADHD, sleep disorders, depression, etc. The misdiagnosis leads to treatment that is not suited to the problem, thus causing the children prolonged suffering.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           The study was led by Prof. Shai Efrati of the Sagol Center for Hyperbaric Medicine and Research at Tel Aviv University and Shamir Medical Center (Assaf Harofeh), Dr. Uri Bella and Dr. Eli Fried of Kaplan Medical Center, and Prof. Eran Kotzer of Shamir Medical Center. The results of the study were published in the journal Scientific Reports.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           “The objective of our study was to determine how many children in Israel suffer from persistent post-concussion syndrome,” says Dr. Fried of Kaplan Medical Center. “The children participating in the study arrived at the emergency room with mild head trauma and, after staying overnight for observation or being sent for a CAT scan of the head, they were discharged to go home.”
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           Prof. Efrati of Tel Aviv University states: “Persistent post-concussion syndrome is a chronic syndrome that results from micro damage to the small blood vessels and nerves, which may appear several months after the head injury, and therefore is often misdiagnosed as attention deficit disorders, sleep disorders, depression, etc. There are cases where children report headaches and are diagnosed as suffering from migraines or, for example, children who report difficulty concentrating and the doctor prescribes Ritalin. Unfortunately, these children continue to suffer for many years from various disorders and, instead of treating the real problem, which is the syndrome, they receive treatments that usually do not solve the problem.”
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           The study examined 200 children who suffered from a head injury and who were released from the emergency room after the need for medical intervention was ruled out. The researchers tracked the subjects for a period between six months and three years from their date of discharge and found that about one in four children released from the emergency room suffered from the chronic syndrome.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           “It should be understood that the consequences of brain injury during childhood continue throughout life,” says Dr. Uri Bella, Director of the Pediatric Emergency Room at the Kaplan Medical Center. “Loss of any brain function will prevent the child from realizing his or her potential in education and in social life.”
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           Unlike damage to large arteries and noticeable damage to brain tissue, with a minor head injury, the damage is to the small blood vessels and neurons — and it is not detected on CAT scans of the head or on regular MRIs. Diagnosis of the syndrome requires long-term monitoring of the manifestation of symptoms as well as the use of imaging and functional tests of the brain. According to the researchers, the alarming findings demonstrate that changes in the approach are needed to be monitoring and treating these children.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           “The purpose of an emergency room diagnosis is to determine whether the child suffers from a severe brain injury that requires immediate medical intervention,” adds Prof. Eran Kotzer, Director of the Emergency Rooms at the Shamir Medical Center. “Unfortunately, the way most medical systems operate today, we miss long-term effects and do not continue to monitor those children who leave the emergency room without visible motor impairment.”
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;

           “Treatment for a wide range of disorders will change if we know that the cause of the new problem is a brain injury,” concludes Prof. Efrati. “Proper diagnosis of the cause is the first and most important step in providing appropriate treatment for the problem.”

      <title>Concussions affect children’s brains even after symptoms subside</title>

           Brain changes in children who have sustained a mild traumatic brain injury, or concussion, persist for months following injury — even after the symptoms of the injury are gone, according to a study published in the December 12 issue of The Journal of Neuroscience. The findings highlight the potential benefit of using advanced imaging techniques to monitor recovery in children following concussions.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           Despite growing concerns over the risk of concussions in youth, the majority of research examining their effects on the brain has involved adults. These studies suggest that, among other things, concussions alter the brain’s white matter — the long fibers that carry information from one area of the brain to another. Some recent data suggest the developing brains of children may be more vulnerable to the effects of mild traumatic brain injury.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           In the current study, Andrew Mayer, PhD, and colleagues at the Mind Research Network and the University of New Mexico in Albuquerque, New Mexico, specifically studied older children (ages 10-17) with mild traumatic brain injury. They found that structural changes in the children’s white matter seen about two weeks after the injury remained evident more than three months later despite the disappearance of symptoms related to the injury.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           “These findings may have important implications about when it is truly safe for a child to resume physical activities that may produce a second concussion, potentially further injuring an already vulnerable brain,” Mayer said.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           The researchers conducted cognitive testing and used an advanced imaging technique known as diffusion tensor imaging (DTI) to examine the brains of 15 children who had recently (within 21 days of injury) experienced a concussion and 15 unaffected children. In the brain, DTI specifically images white matter. During a follow-up visit approximately four months post-injury, scientists repeated cognitive testing and imaging.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;

           Initial testing revealed that children with the mild brain injury had subtle cognitive deficits and changes in white matter compared with healthy counterparts. While the children did not report symptoms of the injury during the follow-up visit months later, DTI revealed that the structural changes to the brain remained.

  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           “The magnitude of the white matter changes in children with mild traumatic brain injury was larger than what has been previously been reported for adult patients with mild traumatic brain injury,” Mayer said. “This suggests that developmental differences in the brain or the muscular-skeletal system may render pediatric patients more susceptible to injury,” he added.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           Based on the imaging data collected during the study, the researchers were able to accurately distinguish the brains of patients who had mild traumatic brain injury from those who were healthy 90 percent of the time. Such findings suggest DTI, which does not require the use of ionizing radiation, could one day be used to diagnose the injury and to better characterize the recovery process in the brain.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           Christopher Giza, MD, an expert on developmental brain injury from the Mattel Children’s Hospital and the Brain Injury Research Center at the University of California, Los Angeles, who was not involved in the study, noted that while the number of patients in the study was small, the findings mark “an important step forward in our understanding of the effects of mild traumatic brain injury on the developing brain.”

  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           “Further work is needed to determine whether the changes in white matter present at four months represent a prolonged recovery process or permanent change in the brain,” Giza added. “Determining the duration of the structural changes, and whether these changes have clinical implications, remain critical areas for future studies,” he said.

           Different types of brain damage caused by a concussion may lead to similar symptoms in children, according to research led by McGill University. A new way of studying concussions could help develop future treatments.

    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           While most children fully recover after a concussion, some will have lasting symptoms. The findings published in eLife help explain the complex relationships that exist between symptoms and the damage caused by the injury.
          &#xD;
    &lt;/span&gt;&#xD;

  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;

           The researchers found that certain combinations of brain damage were associated with specific symptoms such as attention difficulties. Other symptoms, such as sleep problems, occurred in children with multiple types of injuries. For example, damage to areas of the brain that are essential for controlling sleep and wakefulness could cause challenges with sleeping, as could damage to brain regions that control mood.

           To do this, they examined how damage to the brain resulting from concussion affected its structural connection network, known as white matter. They then used statistical modelling techniques to see how these changes related to 19 different symptoms reported by the children or their caregivers.

           Analysing symptoms may advance treatment

           “Despite decades of research, no new treatment targets and therapies for concussions have been identified in recent years,” says lead author Guido Guberman, a Vanier Scholar and MDCM Candidate at McGill University. “This is likely because damage to the brain caused by concussions, and the symptoms that result from it, can vary widely across individuals. In our study, we wanted to explore the relationships that exist between the symptoms of concussion and the nature of the injury in more detail.”

    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;

           Guberman and his colleagues analysed data collected from 306 children, aged nine to 10 years old, who had previously had a concussion. The children were all participants in the Adolescent Brain Cognitive Development (ABCD) Study.

  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;

           “The methods used in our study provide a novel way of conceptualising and studying concussions,” says senior author Maxime Descoteaux, a Professor of Computer Science at Université de Sherbrooke. “Once our results are validated and better understood, they could be used to explore potential new treatment targets for individual patients. More broadly, it would be interesting to see if our methods could also be used to gather new insights on neurological diseases that likewise cause varied symptoms among patients.”

      <title>Research brings hope for spinal cord injury treatment</title>

           Scientists from the University of Birmingham have shown an existing drug may reduce damage after spinal cord injury, by blocking the inflammatory response in the spinal cord.

    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;

           Their research, published today in Clinical and Translational Medicine, demonstrates that AZD1236, a drug developed by AstraZeneca, can significantly reduce ‘secondary damage’ caused by the body’s response to spinal cord injury (SCI).
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;

           Researchers led by Professor Zubair Ahmed, Professor of Neuroscience and lead for the Neuroscience and Ophthalmology Section at The University’s Institute of Inflammation and Ageing, used animal models to demonstrate that AZD1236 can promote significant nerve regeneration, with a dramatic 80% preservation in nerve function following spinal cord compression injury.
